Dr. Albert West, the protagonist of the game, joins MPO after being personally invited by Mr. Murray. He had been working on a cure for Alzheimer's disease, hoping to save his wife Jennifer from dementia. However, his efforts are abruptly interrupted when a lab accident leads to an accusation of murder and a subsequent prison sentence.

The game begins over a year into Dr. West's prison sentence, with the guards mysteriously vanishing. As Dr. West navigates through the prison, he is about to uncover the truth behind their disappearance and the horrors that await him on Mars.
